Import a questionnaire into Simplycure

Export a questionnaire as a PDF or .docx from Google Docs, Google Forms, Tally or Typeform, then import it into Simplycure.

This article shows you how to export your patient questionnaire as a PDF or .docx from your usual tools, so you can then import it into Simplycure.

Accepted formats: PDF and Word (.docx).

Unsupported formats: Excel (.xlsx), PowerPoint and links to online forms. Export as PDF or .docx before importing.

From Google Docs

File → Download → Microsoft Word (.docx) or PDF Document (.pdf).

From Google Forms

  1. Open your form in the Google Forms editor.

  2. Click the ⋮ (three dots) menu at the top right, next to Publish.

  3. Select Print.

  4. In the print window, choose Save as PDF as the destination.

  5. Click Save.

From Tally

  1. Open your form in the Tally editor.

  2. Open your browser's print settings (⌘P on Mac, Ctrl+P on Windows).

  3. Choose Save as PDF as the destination.

  4. Click Save.

From Typeform

Typeform does not offer a native PDF export. The free tool Koppie lets you export all your questions into a single file:

  1. Go to servalian.com/tools/koppie.

  2. Connect your Typeform account via the Connect with Typeform button.

  3. Select the form to export.

  4. Choose the PDF format and download the file.
